2022 Annual Street Listing/Census

2022 Annual Street Listing/Census forms are being mailed to every household in Carlisle beginning December 13th. 

Please expect this form and open upon receipt. It is a LEGAL DOCUMENT that Massachusetts General Law requires. Failure to respond shall result in removal from the active voting list and may result in removal from voter registration rolls. 

Review the front of the form carefully and make any corrections as needed. Even if you do not need to make corrections, the form is still required. Sign and date the bottom. (Detailed instructions can be found on the back of the form.) Use the self-addressed envelope to return to our office by mail or you may drop it off in person or in the drop box to the left of the front doors of Town Hall.

Below the instructions is an application for 2022 dog licenses. All dogs six months and older must be licensed annually. If a dog/dogs live in your household, fill in the information on the application and include payment, the current rabies certificate, and a self-addressed envelope with $0.79 (non-machinable) postage when returning your census form. Payment can also be made online, but our office still needs a current rabies certificate.
