2. Use the below form to order your initial 2026 Transfer Station (TS) permits (stickers) for check payment only. Do not use this form for online payment. 

   Each household is allowed two permits. 

   Additional permits are available after review by Town Staff or the Transfer Station Action Committee. If you have already purchased at least one 2026 TS permit, use the 2026 Additional or Replacement Transfer Station Sticker Form

   After submitting this form, mail or drop off check payment to Town Hall, 66 Westford St, Carlisle, MA 01741, ATTN: TS Permit. Checks payable to "Town of Carlisle". Include "TS Permit" on the memo line.
