  2. Contact Fire Prevention Office for fire and carbon monoxide requirements. 
  3. Contact Planning Board to inquire about any restrictions
  4. Contact Conservation Commission regarding wetlands and land use issues. 
  5. Two sets of detailed plans.
  6. Completed residential application with homeowner signatures.
  7. Certified plot plan by a surveyor.
  8. HERS Preliminary Report.
  9. Workers Comp Insurance and Liability Insurance Certificates.
  10. Construction Supervisors License. 

Addition or Alteration
